Pace of the story is constant but the twist is excellent. But how can the protagonist be able to walk free after the last killing and even kills the physio who is just trying to help her.
Plot summary
Shugo Hayami is a doctor at a nursing hospital, which was once a psychiatric hospital. One evening, when Shugo is on night duty, a man in pierrot mask breaks into the hospital and he is not alone. He is with Hitomi Kawasaki, a university student who has been shot by the masked man. The masked man tells Shugo to treat Hitomi. He also takes hostage the 64 people in the hospital. Soon, Shugo and Hitomi try to escape from the hospital and they encounter mysterious cases one at a time.

Perplexing ending
Confusion Substituting For Suspense.
The story line of this suspense thriller concerns victim revenge on an illegal (and lucrative) organ harvesting and transplanting business based in a malpractice factory masquerading as an inconspicuous, backwater hospital. So far so good. Unfortunately, the execution of this novel and interesting plot device is pretty much on life support (if not DOA--dead on arrival). There is a boat-load of padding with lots of repetition. (Even the surprise ending--which is incredulous--never seems to end due to intense padding.) Excessive use of the onion-pealing process to gradually reveal who the characters really are and what seems to motivate them makes for chronic audience confusion and, ultimately, loss of interest and boredom. Direction is uneven and so is the acting with actors generally delivering perfunctory performances due in part to being saddled with silly dialogue (one writer is also the Director). Cinematography and lighting are okay. So is the music. Subtitles suffer from a lack of editorial downsizing and an often ludicrous presentation pace. Not especially recommended. Too many plot devices
The concept of this film was interesting but too many plot devices were used to move the story along. I don't care for movies where implausible events have to occur in order to advance the story. The acting was decent but the ending was really stretched out.
